Yes, I have!  The system (a 3B1 w/ 2meg RAM & the 67 meg HD) was still
under warantee.  After replacing the power supply and the same thing still
happened, AT&T replaced the 2-speed single muffin fan with a single speed
fan and I've since had absolutely NO more problems.
