First name basis is a good aproach, but it doesn't work in formal
situations like addresses, etc.  In California, and much of the West
coast FNB is quite prevalent, but I must say it took me a long time to
get used to it after many years of East coast stuffiness.  Also FNB does
not solve the problem of women losing their maiden names after marriage.
I find this much more annoying than the Miss./Ms./Mrs. problem.
