The answer to the question of how to strike out with no pitches thrown:
(the net answer of skip in the batting order is correct but I was looking for:)

If a batter is due up and does not enter the batter's box, the ump asks
him to step in the batter's box.  If he does not step in, the ump
at his discretion, calls a strike, without the pitcher throwing the ball.
If the batter still does not step into the box, the above rather grim
scenario may be repeated twice more to strike the bum out.

This could be the answer to Dave's question of winning a game without
throwing a ball, but alas, it isn't...............
