In (I believe) 'Let that be Your Last Battlefield' is the only
time that the self destruct sequence is actually initiated.  It
proceeded with Kirk, Spock, and Scotty (i.e. the captain, first
office, and chief engineer), in that order, being verified by
the computer (I think by voice) and then giving a sort of numerical
password (orally).  Once all three had been confirmed and the
codes checked out, a 30 second countdown was started.  Kirk said
that it could be stopped at any point up to T-5 seconds, after
which ''no command in the Universe can stop it.''  It was, of
course, halted at T-6 seconds.
