For a detailed answer, see the "README" entry that was last posted to
comp.mail.maps in early July. Here's a short extraction from that file:

#N UUCP name of site
#S manufacturer machine model; operating system & version
#O organization name
#C contact person's name
#E contact person's electronic mail address
#T contact person's telephone number
#P organization's address
#L latitude / longitude
#R remarks
#U netnews neighbors
#W who last edited the entry ; date edited
#
sitename .domain
sitename remote1(FREQUENCY), remote2(FREQUENCY)

You should fill in as much as you can, but omit the "sitename .domain" if
you're only a small private uucp site ( not xxx.com etc. ). I was going
to send the complete file, but your upstream feed is also not in the maps.
You've got to get him/her registered also before your entry will do any
good ...

Send the completed entry to "rutgers!uucpmap" ( they connect to uunet or
gatech or ucsd or ames if that helps ).
