Please help me !!!

I've bought a 360KB 40 tracks SD/DS IBM compatible diskdrive model RFD480
from Ex-Cell-O Corporation (REMEX Division). But I have no documentation
and no wiring diagram of it. I want to connect it to an ATARI ST Computer. 

My first question is about a jumper-block of 4 jumpers (A,B,C,D) and their
functionality. This is not the jumper-block for the device-select signals.

My second question is the more important one. By some tests I've killed a
IC on the position B2. It has 14 pins and the name UHP407 on the frontside
and PHIL 004 54 on the backside. The boardtype is REMEX ASSY 114861-001 REV.
Nobody in Vienna knows the IC. Does somebody know if the IC is compatible to
a known IC. If not where I can buy it or better the Address of the company.
