The fact that format tells you that it is starting at cylinder 2 tells me
that your Mountlist entry for the FFS partition is wrong.  You must tell
Prep where your first partition ends...it ASSUMES that the first partition
begins on cylinder 2.  If you tell prep that the first partition ends on
cylinder 2...then you have to start your FFS partition on cylinder 3.  The
first partition cannot be FFS.
At least that worked for me and I have the same setup as you except that my
drive is an ST251 40 megger.
