Yes, you're missing something.  Smail is a mail routing agent, not a user
agent.  It is not intended to replace Berkeley mail or MH or any other user
agent.  I don't run it, but I believe the way you install it is to put it
in place of /bin/rmail, so that when a mailer tries to deliver mail, it will
invoke Smail instead of simple rmail.
