In order to get this you will need to find someplace that has ACM merchandise
order blanks.  See if you can find a friend that is a member of the ACM and he
might have them or know where you can get some.  Otherwise you can try a local
college library, if they have a computer science program worth its salt then 
they will carry some of the ACM's publications.  The very best solution would
be to find a friend that is not only a member of the ACM but also Siggraph
because the tapes are damned expensive and he/she can order them cheaper.
