I use "Bargain basement" floppies from MEI/Micro or Direct Micro
($.99 and $1.19 in bulk, respectively, I think.)  I would estimate a
failure rate of <4%.  I have had much worse luck with more expensive
disks.

(this is out of a total inventory of about 250 disks, 2/3 of which
are the cheap ones.)
