My drivers, at least, allow negative offsets everywhere.  If you
generate a page header with something like

 \headline={blah blah\hfil\raisebox 8ex{\folio}} % 8ex is a guess

the page number should come out above the top margin, at a `negative'
y position (y=0 => 1 inch from top of page).

Another more insidious problem you may have is with the printers
themselves, which sometimes cannot print within a centimetre or so
of the edge of the page.
