I had a similar problem with one of the first Dan Wesson
.44VH models. Every so often a round would failure to fire due
to what looked like a light hammer fall. I sent it into the factory
and they determined that there was improper headspacing (I didn't know
this could happen on a revolver!). They fixed it and it's worked fine
ever since, although I don't shoot it much.

Another problem you might be having is the primers. A reloader
friend of mine told me that CCI primers are less sensitive than other
primers. So a less-than-heavy hammer fall won't set them off. This
has been my experience, it's something for you to consider.
