	the one i've alway's liked the most is as follows:

	the problem was simple process control, truck pulls
up to a filling station, gravel comes down the chute until
weight 'x' is reached whereupon the gravel flow stops and
the truck leaves.

	well, the program was coded and (i suppose ) tested
with stubs for the real-live chute, but when the first live
run went through, the truck was BURIED in the gravel.

	seems that he had programmed the chute gate to close
when "weight = x" instead of "weight >= x" !!
