Summary: 

I have an object that I would like to have embedded in a block of clear
plastic.  I know it's possible to buy kits for this, but I played with
with them once and I know it's tricky to do a good job.  So I want it
done professionally, if possible.

Normally I would start with the Yellow Pages, but in this case I wouldn't
even know what category to look under.  Can anybody point me to where to
go, or where to look?

(It's an old vacuum tube that is dear to me because I found it bobbing
in the Pacific Ocean while visiting Kaikoura (so look it up).  I've
already cracked it twice -- the first time spoiled the vacuum and
chipped my TV screen, and the second time I had to glue the glass back
together.  So I'd rather there wasn't a third time, and knowing me,
there probably will be one unless I do something.)
