If you're using nroff or something like it, you can define macros.
Depending on a \n at the beginning, let nroff print

	1. the old version,
	2. the new version, or
	3. the old version (if any) between [brackets] and the
		new version (if any) in _italics._

No. 3 is how Boswell did it in his _Life of Johnson._

Then again, maybe Xerox PARC will let you use some of their software ...
