I can only speak for 3.6, but here goes....

There is a special flag inside the command loop that tells whether both
BEFORE and AFTER fuses take place.  BEFORE fuse actions are rings using food,
being confused, having monsters move, having blindness wear off, etc.  In
general, things which elapse time.  Only movement, T,W,P,R,e,q,r,. or z commands
trigger the BEFORE fuses.  *All* actions (including illegal keystrokes, or 
inventories)  trigger the AFTER fuses.

What is on the AFTER fuse list, you ask?  Just the time before the next
wandering monster gets created....

I assume that if you were standing on a scare monster scroll, and worried
about food consumtion, you could hit the i key three million times....
