It happened this year; I saw it on TV.  With only a few seconds left in
the first half, one team (sorry, don't remember the teams) scores and
kicks off.  They try a squib kick; the ball is a short looping kick and
one of the up men calls for a fair catch (on a kickoff!) at his own 45.

They elect to go for a free kick attempt at a field goal.  Everyone was
confused.  Everyone starts to line up at the line of scrimmage for a
conventional field goal.  They finally get straightened out.  The
defensive team must line up ten yards downfield.  The kicker kicks from
the line of scrimmage, but he is not allowed a tee.  He may (and did)
use a holder (beats drop-kicking!).  He missed the attempt at what would
have been a 65-yard field goal.
