With regards to the fact that many women don't mind being called girl:

You have to remember that many women need to have their consciousness
raised as much as men.  They were brought up in the same world.  It is a
very subtle thing we're talking about here, and it is not obvious to
many people, especially those (the majority) who give no thought to the
effect of language on society (or vice-versa).
