Use two club entries to dummy to ruff diamonds.
Enter dummy with the last trump.
Position with east to pitch:

A: east cannot pitch the AD because declarer will pitch a heart on the KD
   and finesse spades.
B: east cannot pitch a heart because declarer will finesse spades and run
   the hearts.
C: so east pitches a spade. Now declarer leads a low spade to the QS. Cashes
   the AS. Enters dummy via the JS. And east is squeezed again. With three
   cards to play, east cannot keep three hearts and the AD.
