I had a problem where the disk was somehow glued (?) to the diskette liner.
I had to caaarrefulllly slice up the jacket, and caarrefulllly pull the 
"cookie" out from the jacket, and insert it the same way into a scrap
diskette liner.  Then it read fine.  I think that the problem was caused
when the disk was shipped, and exposed to very high temperatures.
