The most runs in one game was set back in the 20s or 30s
by the Cubs and the Phillies, with the final being 26-23!
49 runs in one game.  And that`s in 9 innings.  A couple
years ago those 2 teams had another donneybrook, with
the Phils winning 22-21 in 10 innings.  In that game, the
Cubbies were down by 10 or more runs and came back, only
to see Mike "Cub Killer" Schmidt homer in the 10th for
his 4th homer of the game.  

By the by, the most runs in an inning is 17, set by the
Boston Red Sox.  Can`t remember the year, though I suspect
the 30s or 40s.
