I use the shaft of a prick punch.  The one I have is a gently
tapered tool that doesn't abrade the record.  When the record hole
is a bit too small to fit comfortably over the spindle, it's usually
just from the paper being a little off-center, or from a small ridge
left in the vinyl from the stamping equipment.  GENTLE poking with 
the tool is all that's usually required.  You want to push the irregularity
aside without throwing the hole off-center, so don't use drills or files,
etc.

...Of course, compact discs don't present such problems.
