1) How come Savik didn't know to bring Spock back to Vulcan
at the end of ST II? If she did, why did she let Kirk
    leave him on Genesis?

Saavik was raised Romulan, wasn't she? At least, she wasn't raised
Vulcan. When Spock took her under tutelage, one may assume that
he instructed her in Vulcan Discipline, but not yet get around to
complete intruction in Vulcan culture ( although, for some reason,
she did know about Ponn Farr ).

2) Why didn't Starfleet use the prefix code to stop Kirk
    from stealing the Enterprise?

Excellent hole. Indeed, was this not the very circumstance the prefix
was designed for ( regaining control of a ship in "hostile" hands )?
Since the prefix code is a communications/computer function, though,
it may have been sabotaged by Uhura and Scott.

3) What was the timespan of ST II? It didn't seem very long
at all. Recall that the Enterprise had just been refitted.
Why, all of a sudden, is it not going to be refitted? It's
not like it's been out for years, after all, just one short
journey since a major overhaul. Makes the overhaul go to waste.

You're mixing timespans. Enterprise was refitted in STI. When Kirk
returned with refitted Enterprise, they had been out shaking her down.
From that time, in which Nagura was Kirk's superior, and the uniforms
were inferior, we go to STII. A new captain of the
    Enterprise ( Spock ) a new mission ( training vessel ), and a new
superior officer ( Morrow ). That could very easily be anything from 1 to
5 years. It is not unreasonable for an old ship to be refitted to make it
suitable for a trainging vessel ( witness the Battleship USS New Jersey ).

4) When the Enterprise was entering Spacedock, and they showed
the lounge and its patrons ooo'ing and ah'ing, they showed
one woman's face; she was shaking her head. Who was she, and
what was her purpose in the movie? I thought then that she
was some higher-up, disgusted with what Kirk had done to the
ship, and about to take away his command; I was surprised to
find out she had no further part in the movie. It left sort
    of a hole.

That was the cameo appearance of transporter chief Janis Rand ( Grace Lee
    Whitney ).
