My 79 Renault LeBomb made a similar sound: then I noticed that the exhaust
pipe leaves the manifold, passes through a hole in the sheet metal to the
left-front fender well (!) where the first of 2 mufflers is located (!) and
then out under the car to the second muffler at the rear.

It seems that under load the engine shakes just enough to cause the pipe
right before the first muffler to bang against the sides of the hole where
it passes through to the fender well. It makes an infernal racket. I had
the pipe bent so that it passed through dead-center, and the noise went away.
