The full story of this oddity comes down to an "error" by someone
in the editing process of this episode.  Close-ups of the "bad" Kirk
were reversed (left-to-right flipped) to make his evil nature
subliminally apparent.  When filming, they put the scratches on the
"wrong" side so that when flipped Kirk would appear as planned, but
when editing, someone forgot to flip it (or flipped it back) because
Kirk would have been reversed otherwise (which they naturally assumed
was an "error").  This accounts for at least one error like this in
"The Enemy Within"--there may be others where a make-up or continuity
person slipped up.
