I have never seen a bar with a rotating light that went off when someone
smokes. However there is (or perhaps was) a bar in Milwaukee called the Safe
House. In the Ladies Restroom it had a foldout of Burt Reynolds with a fig
leaf covering the appropiate area. The fig leaf was rigged so that if someone
lifted it up it would flash lights and sirens outside the door.Newcomers to
the bar generally were not told about this until after they returned and were
wondering why everyone was smiling.

That place had a lot of other gimmicks all done around the theme of being
a "Safe House" for spies. I haven't been there for about five years. Does
anyone know if it still exists?
