Try typing "dirs" or "set" or some such built-in command to the shell, to see if it works ok when there is no disk activity required. You may have a flaky attention summary bit or be missing interrupts or something. If necessary you can put a watchdog timer in the disk driver to poke the controller if an interrupt hasn't happened after a certain length of time. System III does this.
