Now, I do have the expansion port cover, and would not mind putting the
RCA's there, however I do like his spot for mounting. He had to pull it
from the inside, which is more difficult, and I think I want to go with
the simpler method and pull it from the expansion port, but make the RCA
plugs look like his setup but on the expansion cover.

Now this shows which pin has auido and video. The only thing I am
confused about is the ground. Should each audio and video channel have
it's own ground connection to the shielding of the cable, or can I just
split a ground connection? Does it matter which ground I use for any of
the RCA's? 

And finally, I need some clarification on what this means:

Those are external line head screws.  You need the 4.5mm security bit 
(typically called a "game bit").  MCM Electronics 
(http://www.mcmelectronics.com/) sells these.  I bought p/n 22-1150, but 
it requires a standard 1/4" hex driver.  You may want to poke around on 
their site looking for the most suitable product for your situation.

yeah, i bought mine from MCM. They charged alot for
shipping though. Sometimes you can get they from ebay.
Its the security gamebit. Same one used for super
nintendo games i believe.

Several years ago, I called the company that NEC was working with seeing if
I could track down any info on these Discis CDs.  I was basically told that
no, none were ever released, and that any agreement that they had with NEC
was no longer going to be honored.  It sounded like some bad blood had flown
between the companies.

ok well i think that settles that, can anyone now confirm if the bikini
girls range of cds ( 3 i think ) were US or Jap? i have all three, even
though the covers are in english i believe ive seen them listed as jap cds?

Best information we were able to round up when they came out is that they 
were made in Texas by "Excite Software."  I cannot confirm this, nor was
I ever able to find any info on an 'Excite Software' anywhere in Texas (or
in the US, for that matter), so I do not know if that info is authentic or 
not.  I can confirm that the info came from an NEC/TTI insider (at the 
time), but I do not know his source for the info.
