I think I am having the same problem with my rev 4 II+ that is described here:

My video is shifted about 1-2 inches to the left. When using the same monitor with an unenhanced IIe, I get perfect video. I opened the monitor and did not find any adjustments for horizontal position or centering. There is a width adjustment, but it does not seem to move the image more than a few millimeters. FYI the monitor is an A2M2010.

Anyone know of a good remedy?

My Rev 4 will generally not work with a composite video monitor. It does actually work with one of my Sony CRT based TV sets when connected directly through composite video IN. And, it also works on the B/W Sanyo monitor - those monitors you generally see in many earlier Apple II brochures. Otherwise, no luck with most other non-Apple monitors I've tried.

But, the Rev 4 does work on any TV set through an RF modulator running off VHF channel 3. Same thing goes for my Sinclair 1000 - no monitors, but success when using an RF modulator on a TV set.

I also have a Rev D. which is the RFI version. And, it works with EVERYTHING!

I went ahead and turned the two convergence rings and I was able to get this II+ video to display in the center of the tube for my monitor!

So the problem must have been the same as the previous thread that I linked to above. It looks pretty good.
