When I insert a floppy disk it immediately ejects the disk. Chime is heard
when I power on, ? disk icon appears, I insert disk & it comes right back
out and gives me the X icon.
I've removed the drive (it stopped working in 1994 and I have just now
opened it) and have greased all moving parts, cleaned the read head, and
e'things appears ok.
Sometimes I do hear the "click of death" upon powering up, other times not.
I have an external Apple Drive 800K external which *will *boot but doesn't
act 100% as it should (for instance, it won't eject on command but it will
get the printer to print (25 years later and it STILL prints!)
Any ideas/suggestions/thoughts?

Yes, it's the original 400K internal floppy drive.

Right, but is it a 400K disk as well? The 400K drive will spit out 800K and
1.44FDHD disks without even thinking.

Yes. The disks I am using for testing are the original System Disk and
Original MacWrite/MacPaint disks, along with several other single-side
3.5s, some which were new and never used before. (formatted for Mac).

Are you able to read these disks in another Mac? It may be that they have become corrupted. If the drive cannot read the system file, it will eject the disk.
Also, there are two switches at the front of the floppy drive. One on each side, at the front. One detects the disk inserted, the other detects the read/write notch. Are these working correctly and not sticking? You may need some contacts cleaner to remove any gunk.
While a failed floppy controller is a possibility, the more likely culprit is a mechanical problem.
Try to read the disks in a Mac with an 800k drive as it can read both 400k and 800k. 1.4 superdrives read only 800k and 1.4MB disks.
A USB floppy drive will only read 1.4MB disk.
You may also want to give your external floppy a light overhaul and check the switches on that also.

I read and write discs from my LC III. I've even had a system disc mailed
to me by another Rescue My Classic Mac
Unfortunately, I don't have an external drive...yet

Yes, I do have an external floppy and it IS able to read the old MacWrite
and another disk I tried. I was even able to print to the original dot
matrix printer.
I've had various reactions from the screen after various cleanings and
small adjustments to the internal drive. Now, it's just spitting them out.
But I've had the screen do nothing--as if I never inserted a disk; I've had
it go through the motions with a happy face only to go black with the "X"
and I've had it go straight to the black "X" and have had to manually eject
it.
I've learned a lot about the inside of both the internal drive (I've taken
it out at least 10 times) and the rest of the 128K, but I've never heard
nor read about the 2 switches at the front--thanks for that! I gave it a
good cleaning and lube job but I can't say specifically that I addressed
these areas.
It's still out of the case now, so I'll give that a try. If it doesn't
work, I'm going to send it to a guy in MA who has a half dozen laying
around. He'll check my to see if he can fix it (I'd like everything to stay
original to the machine if possible) or he'll send me one he's repaired and
knows is working for $100. I think that's a fair deal.
